Non Union Resident Care Liaison
1 year contract Beginning September 2019
Reporting to theDirector, Community Outreach and Programming, in this temporary(1 year)full time position (5 days/week, varied hours/days); you will be primarily responsible for ongoing support, counselling, education, as well as information to residents, family members and Substitute Decision-Makers (SDM).
As a member of the multidisciplinary team, you will work in collaboration and consultation with other health care disciplines to promote the optimal psychosocial functioning of the residents and their families during the transition to long-term care and palliation. You will advocate for the resident and may work to resolve problems related to institutional living and will refer individuals to community resources asnecessary. You may assist residents, families, Powers of Attorneys, SDMs and Guardians in working through legal, financial, and basic care (e.g. comfort allowance) issues.
Candidates will possess a post-secondary education in Social Work, Social and or Human Services with two or more years’ experience in a geriatric setting preferably Long Term Care or relevant education andexperience deemed acceptable.Knowledge of the MDS/RAI system for long term care, bilingualism and group work experience considered an asset.
